NEW THIS YEAR: Athletic Code of Conduct will be found on under Athletics Forms on the MACS website. The signed agreement needs to be turned into the Athletics Office before you can began participating. 

Athletic Team Registration

ALL PAPERWORK MUST BE SUBMITTED PRIOR TO PARTICIPATION

AT ANY LEVEL, IN ANY PRACTICES... NO EXCEPTIONS

  1. Each student-athlete must have a completed WIAA Sports Physcial Card on file at McDonell Central Catholic High School. This can be downloaded during the registration process and a copy must be dropped off at the Athletic Office.
    1. Note: Physical examinations taken April 1 and thereafter are valid for the following two school years; physical examinations taken before April 1 is valid only for the remainder of that school year and the following school year. 2021-22 sports physicals must be dated 4/1/2021 or later.
    2. WIAA Alternate Year Cards are to be completed every other year. This can be downloaded during the registration process and a copy must be dropped off at the Athletic Office.
  2. Parents and student-athletes should read the Athletics Code of Conduct found on the athletics website and RETURN A SIGNED COPY OF THE AGREEMENT TO THE ATHLETICS OFFICE.
  3. Emergency Medical Information
  4. Athletic Fee Payment, payable to McDonell Area Catholic Schools. This should be paid at the McDonell Central Catholic High School main office. Coaches will receive composite list of whom has paid. 
  5. Concussion/Head Injury - Participation and Parental Disclosure and Consent Document.
  6. A parent/guardian and student-athlete must attend one Sports Meeting per school year. This years meeting is on August 1th, 2022 at 6 p.m.
